Pre-Participation/Medical Forms
Attention Student-Athletes and Parents/Guardians:
Effective for the 2025-2026 school year, Students in Grades 7-12 interested in interscholastic athletic participation must complete and submit the following:
Physical Examination and PPE Medical Eligibility forms, click on the name below:
Physical Examination Form - Physical examinations must be completed by a personal family physician. These pages can be retained by your doctor or kept at home.
PPE Medical Eligibility Form - This page must be handed into the athletic office. We must have a current and valid physical (no more than 365 days old) on file.
It is crucial to indicate the grade your child is entering in September on the Athletic PPE Medical Eligibility form and the sports they would like to participate in. Summer workouts and the strength & conditioning programs start on June 16, 2025.
Complete and submit the NJDOE Health History Questionnaire form for every sport season located under the Forms section in the Genesis Parent Portal. For fall sports it will be released mid-July.
Submit the following Athletic waivers electronically through Community Pass which opens mid-July of current school year. Annual athletic/activity fee is $75.00 (HS) and $50.00 (MS):

In order to be guaranteed clearance for fall sports the forms must be submitted no later than Friday, August 8, 2025.
The completed PPE Medical Eligibility form can be submitted in the following manner:

Scan and email clear and legible form to Jill.Dunican@riverdell.org.
Your compliance with this procedure will ensure accuracy and expedite the process of officially clearing the student to participate.
All athletes must practice a minimum of 6 days before participating in scrimmages as per NJSIAA rules.
